Rodeo Waiver and Forms

Each and EVERY competitor must sign a waiver before being allowed to compete. If you are over 18, you will need to complete and sign the waiver yourself. Any competitors under the age of 18 must have the waiver completed and signed by a PARENT or LEGAL guardian. If the parent or legal guardian will not be present at the rodeo, the form MUST be notarized.
